১। নির্ধারিত ফরমেটে আবেদনকারীর ছবি

• ছবিটি বর্গাকার হতে হবে।
• ছবির ব্যাকগ্রাউন্ড হালকা এক রঙের হতে হবে।
• ছবির সাইজ 100 কিলোবাইটের বেশি হতে পারবে না।

২। আবেদন ফি ১২০০.০০ (এক হাজার দুইশত) টাকা এবং উক্ত ফি-এর উপর সর্বোচ্চ ৩.৫% সার্ভিস চার্জ (৪২.০০ টাকা) SSLCOMMERZ-এর মাধ্যমে আবেদনকারীকে নির্ধারিত সময়ের মধ্যে পরিশোধ করতে হবে।

৩। পছন্দের ক্রমানুসারে কমপক্ষে ৫ টি পরীক্ষাকেন্দ্র সিলেক্ট করতে হবে।

৪। আবেদনকারী বর্তমানে কোন শিক্ষা প্রতিষ্ঠানে অধ্যয়নরত থাকলে সেই শিক্ষা প্রতিষ্ঠানের নাম ও অধ্যয়নের বিষয় সংক্রান্ত তথ্য প্রদান করতে হবে।
