১. আগ্রহী প্রার্থীদেরকে নিম্নস্বাক্ষরকারী বরাবর ডাকযোগে অথবা সরাসরি আবেদন জমা দেওয়ার অনুরোধ করা হলো।
২. কেবলমাত্র নির্ধারিত যোগ্যতা ও অভিজ্ঞতা সম্পন্ন প্রার্থীগন নিজ নাম, পিতা/স্বামীর নাম, মাতার নাম, বর্তমান ও স্থায়ী ঠিকানা, মোবাইল নম্বর, জন্মতারিখ ও বয়স, জাতীয়তা ও ধর্ম, শিক্ষাগত যোগ্যতা ও অভিজ্ঞতা উল্লেখপূর্বক পূর্নাঙ্গ বায়োডাটা, সত্যায়িত সকল শিক্ষাগত যোগ্যতার সনদপত্র, সদ্যতোলা দুই কপি পাসপোর্ট সাইজ ছবি এবং জাতীয়তা সনদপত্রের কপিসহ আবেদন করবেন।
৩. কোন সিডিউল ব্যাংক থেকে ৫০০.০০ (পাঁচ শত) টাকার অফেরতযোগ্য পে-অর্ডার আবেদন পত্রের সঙ্গে শহীদ মনসুর আলী মেডিকেল কলেজ এর অনুকুলে জমা দিতে হবে। অসম্পূর্ণ/ত্রুটিপূর্ণ আবেদন বাতিল বলে গন্য হবে ও কর্তৃপক্ষ কোন কারন ব্যতিরেকে যে কোন দরখাস্ত গ্রহন বা বাতিল করার ক্ষমতা সংরক্ষন করে। দরখাস্তের খামের উপরে ডানপার্শ্বে প্রার্থীর পদের নাম উল্লেখ করতে হবে।
8. প্রতিটি পদে শহীদ মনসুর আলী ট্রাষ্ট প্রবর্তীত বেতন স্কেল প্রযোজ্য হবে।
৫. ১,২,৩,৪ নং পদের বয়স সর্বোচ্চ ৪০ বছর ও ৫,৬,৭ নং পদের বয়স সর্বোচ্চ ৩০ বছর এবং ৮ নং পদের বয়স সর্বোচ্চ ২৫ বছর। অধিকতর যোগ্যতা সম্পন্ন প্রার্থীদের বেলায় শর্তাবলী শিথিলযোগ্য।
৬. নির্বাচনী পরীক্ষা/সাক্ষাৎকার এর জন্য কোন প্রকার টিএ/ডিএ প্রদান করা হবে না।
৭. পত্রিকায় প্রকাশের দশ (১০) কর্মদিবসের মধ্যে আবেদন করতে হবে।
