♦ আবেদনপত্রের সাথে প্রার্থীর পূর্ণাঙ্গ জীবনবৃত্তান্ত, সকল শিক্ষাগত যোগ্যতার সনদ ও মার্কশিটের ফটোকপি (সত্যায়িত), অভিজ্ঞতার সনদ (যদি থাকে), জাতীয় পরিচয়পত্রের ফটোকপি, ওয়ার্ড কমিশনার/ইউনিয়ন পরিষদ/পৌরসভার চেয়ারম্যান কর্তৃক নাগরিকত্ব সনদের ফটোকপি, সদ্য তোলা ৩ কপি পাসপোর্ট সাইজের রঙিন ছবি (সত্যায়িত) সংযুক্ত করতে হবে।
♦ বসুন্ধরা পাবলিক স্কুল এন্ড কলেজ, প্লট-২, ব্লক-এন, বসুন্ধরা আবাসিক এলাকা, ঢাকা-১২২৯ বরাবর গ্রেড-১০ হতে গ্রেড-১১ পর্যন্ত পদের জন্য ৫০০/- (পাঁচশত) টাকা, গ্রেড-১৪ পদের জন্য ৩০০/- (তিনশত) টাকা এবং গ্রেড-১৯ ও গ্রেড-২০-এর পদের জন্য ২০০/- (দুইশত) টাকার পে-অর্ডার/ব্যাংক ড্রাফট (অফেরতযোগ্য) করত উহার মূলকপি আবেদনপত্র ও প্রয়োজনীয় কাগজপত্রের সাথে জমা দিতে হবে।
